Please download and fill out the following forms prior to your appointment. You may either type or handwrite, then sign and submit to Vitality Integrative Medicine. Either scan and email to or bring when you come for your appointment.
*Please note that all appointments are subject to a 24 business hour cancellation policy. Clients not showing up or making last-minute cancellations within 24 business hours of the appointment will be responsible for the full charge of the scheduled services. This policy is necessary as we have to block off a large amount of time specifically for the client, during which the doctor can’t be scheduling or seeing other patients. We also need to prepare treatments, such as IV bags, ahead of time.
